Meet… Mr Francisco
He came to our clinic on Friday, 19 January 2020.
47 years ago, Sr. Francisco was at the beach with his 6-month old daughter. A large wave surprised them and suddenly his little girl was being swept away. Of course, Sr. Francisco reached to save her. The force of the wave dragging his daughter away injured his right shoulder. During the next two years, his shoulder pain worsened. Previous treatments over the years have not helped.
For the past 45 years, the pain stopped him – from raising his arm above shoulder level. Diagnosed with Rotator Cuff Tear and Impingement Syndrome.

The Shoulder Joint is the most complex and moveable joint in the body. There are 18 muscle attachments to the Shoulder Blade!
A muscle normally has 2 jobs… managing 2 opposite movements.

The 4 Muscles of the Rotator Cuff
- Supraspinatus – raises the arm out to the side, controls the arm when being lowered
(Supra – The most commonly injured muscle) - Teres Minor – lowers the arm to the side, controls the arm when being raised out to the side
- Infraspinatus – rotates the arm outward, controls and slows the arm when rotating inward
- Subscapularis – rotates the arm inward, controls and slows the arm when rotating outward

Causes of Rotator Cuff Injury / Tear
Injury or Tear to the Rotator Cuff can occur during nearly ANY movement of the arm. You need an Expert to correctly diagnose and give proper Treatment.
Rotator Cuff Injury/Tear can be caused by (to name a few), 1) Trauma/Injury and 2) Wear and Tear:
1. Trauma
- Overhead activities (carpentry, tennis, swimming, lifting boxes or luggage)
- Turning the arm inward or outward (tennis, closing/opening a door, a fall)
- Suddenly having your arm Pulled or Twisted (such as your dog pulling on the leash)
- Rotator Cuff Injuries / Tear can happen from a 1000 different things,
and even from just Sleeping on the shoulder!
2. Wear and Tear
- Newest research is showing that the Rotator Cuff Muscles act like ropes and pulleys to lift, raise and turn our arm. Because the Shoulder Joint is THE most mobile joint in our body, the Rotator Cuff Muscles get pulled in many different directions and rub around the shoulder blade and arm bone (humerus). This constant rubbing causes “fraying” of the muscles and tendons, much like a rope gets frayed when constantly rubbed.
- The research also shows that “wear and tear” is a normal process that is just going to happen to most of us as we get older. At age 40, 40% will have a tear in the Rotator Cuff Muscle. Age 50, 50%. Age 60, 60%, and so on. This is like a ticking time bomb just waiting for something to happen for a full-blown shoulder problem.
Symptoms
Because there are 4 muscles of the rotator cuff… symptoms of an injury or tear can vary widely. In general, your symptoms may be any combination of pain, achiness, weakness, limited movements, painful sleep.
Your symptoms, what you can or can not do, is very important in arriving at a correct diagnosis.
Diagnosis
Because of the complexity of the Rotator Cuff, a proper Expert Orthopedic exam is critical to properly diagnose your injury.
Your Symptoms plus an Orthopedic Exam is the BEST way to correctly diagnose most Rotator Cuff Injuries / Tear.

New research is showing that advanced imaging studies are of little or no help in the early stages of care and treatment of the Rotator Cuff Injury / Tear.
